Alara provides physician-ordered home health services across Southern Nevada. What that looks like depends on the patient’s condition, function, physician orders, and applicable authorization.
We have particular experience with medically complex patients and federal programs, but the starting point is the same for everyone: understand the person in front of us and what they need at home.

Skilled nursing
In-home nursing assessment and skilled care, including medication management, injections, infusions, wound care, disease management, and other nursing services included in the plan of care.
Home health aides
Hands-on help with personal care and activities of daily living when included in the patient’s plan of care, with nursing supervision as required.
Physical therapy
Assessment and treatment focused on strength, mobility, balance, endurance, transfers, fall risk, and safer movement at home.
Occupational therapy
Help with everyday function — bathing, dressing, household tasks, energy conservation, adaptive strategies, and safer independence at home.
Medical social work
Support with family, psychosocial, community-resource, and practical issues affecting the patient’s ability to receive care at home when included in the plan of care.
The first job is understanding how the patient is actually doing.
A diagnosis list cannot tell us whether someone is becoming less steady, getting short of breath walking to the bathroom, struggling with medications, or needing more help than they did a month ago. The initial assessment gives the care team a real clinical baseline.
Assessment
The nurse evaluates the patient’s current condition, function, safety, medications, symptoms, and other findings relevant to the ordered care.
Plan of care
Care follows the physician’s orders and the patient’s plan of care, including the disciplines, frequency, and goals that apply.
During care
When something meaningful changes, the team can reassess, communicate with the physician, and determine what needs to happen next.
Federal programs
For White Card and other federal-program patients, Alara also coordinates the home-health documentation and authorization work that applies to the services we provide.
Sometimes the difficult part is how everything fits together.
A patient may have several physicians, multiple diagnoses or accepted conditions, medications, therapy needs, family caregivers, and changing function. The plan still has to make sense as a whole.

When a physician orders care for a covered condition, these programs generally pay for it in full. We confirm what applies to you and handle the authorizations.
